Job Description:
Seeking Nurse Practitioners (PRN) for home risk assessments,
including new grads. Conduct comprehensive adult/geriatric
assessments in clients' homes, skilled facilities, and community
settings. Evaluate health status, medical history, medication
adherence, and social factors to improve care and identify chronic
conditions. Collaborate with physicians, case managers, and
caregivers, providing follow-up and documentation per protocols.
Flexible scheduling including days, evenings, nights, and weekends.
Earn $2,000-----"$10,000/month based on assessments completed.
Require current RN and NP licensure, board certification, BLS/ACLS,
and ability to travel locally. New grads encouraged; experience
preferred. Benefits include mileage reimbursement and 401(k).
Travel required to patient homes within Tyrone, PA.
