Job Summary: Ongoing interdisciplinary assessment of the patient. Provides skilled therapy services per physician orders, the patients plan of care, agency policies and professional standards. Performs all aspects of patient/client care in an environment that optimizes safety. Ensures that changes in patients clinical status are communicated to other healthcare team members and agency office staff in a timely manner. Coordinate patient care and participate in case conferences to discuss multidisciplinary team patient progress, plans for continued care, new problems, etc. Educates and counsels patients, their families/caregivers, and other staff as appropriate, in appropriate speech therapy modalities. Provides, modifies, or discontinues physical therapy modalities based upon an ongoing assessment/reassessment of the patients clinical status and in accordance with physician orders Documents all care and services provided according to professional standards and agency policy and procedures. Instructs patients in proper in-home exercises and therapies. Educates patients and their families about the recovery process and the challenges they will face. Identify and meet patients goals and needs Provide quality, personalized and evidence-based care and proven interventions Promote clients healthy lifestyle by improving strength, flexibility, balance, and coordination The ideal candidate will have: Proven work experience as a Physical Therapist Assistant Intensive education and clinical expertise Ability to manage patients with different types of personalities Communication skills Interpersonal skills Pleasant bedside manner Motivation skills Time management Complex problem-solving skills Persuasion skills Persistence Service-oriented Instructing and teaching skills Basic computer skills EDUCATION/EXPERIENCE Licensed as a Physical Therapist Assistant Two years experience as a Physical Therapist Assistant. Previous experience in the home care or outpatient therapy industry preferred. CERTIFICATIONS, LICENSURE, & REGISTRATION Current CPR Certification Must maintain current drivers license and auto insurance
