Job Description:
Swedish Medical Center, a pioneer institution under HealthONE,
is looking for a highly skilled and compassionate RN Case Manager
PRN to join our dynamic team. In this role, you will be integral in
managing patient care with a focus on enhancing the quality of
patient management, maximizing satisfaction, and promoting cost
effectiveness. The successful candidate will be responsible for the
comprehensive assessment of patient and family needs, planning of
targeted transition and discharge, implementation of care plan, and
evaluation of outcomes. Working in collaboration with physicians,
nurses, and other healthcare professionals, you will ensure the
continuity of patient care and optimal patient outcomes. Evaluate
and assess patient needs and establish effective care plans.
Coordinate with healthcare team members to facilitate interventions
and follow-ups. Adjust patient health plans based on evaluations
and in consultation with medical professionals. Provide direct
support and education to patients and families regarding health
care management. Ensure resources are utilized appropriately to
facilitate optimal patient outcomes. Maintain up-to-date patient
records and case notes. Monitor patient progress and make necessary
adjustments to the care plan. Participate in interdisciplinary team
meetings to review and optimize patient care plans. Strong clinical
judgment and critical thinking skills. Excellent coordination and
communication abilities. Ability to manage multiple case loads with
varying needs. Proficient with healthcare IT systems and electronic
health records. Empathetic and patient-focused approach. Medical
Insurance Dental Insurance Vision Insurance Life Insurance 401k
Current Registered Nurse (RN) license. Certified Case Manager (CCM)
certification preferred.
