Description and FunctionsOpen Until FilledGENERAL DESCRIPTION:This position serves as the lead for the Housing Opportunities for Person with AIDS (HOPWA) grant funding and the day to day operations for this grant. This position will work closely with public health nursing offices statewide, local housing authorities, local community action agencies, and others to coordinate housing and supportive services for HIV positive individuals. This position will determine client housing eligibility and develop and maintain HOPWA Program policies and procedures. This position will also assist the Communicable Disease Treatment Program Manager with grant writing and reporting.This position acts as the administrator of CDU Treatment Program systems, including but not limited to Web chart (THR) and CAREWare, the client level data systems for Ryan White Part B and C, and HOPWA grants coordinating user access, training, and reporting functions.This position will also establish and maintain clinical quality management measurements and evaluations for all HIV client services. The HIV/AIDS Bureau (HAB) requires clinical measures for all client services. This position will provide technical assistance to primary care providers, case managers, and staff on Clinical Quality Improvement (CQI). This position will also be responsible for quarterly reporting of performance measurement and evaluation of all HOPWA client services.   • This position will help the Communicable Disease Treatment Program Manager write, oversee, implement, and coordinate grant activities of the HOPWA grant to include all housing requirements and supportive services.
  • Work with clients, case managers, local housing authorities, and landlords to secure safe and affordable housing for enrolled HIV positive individuals and their families.
  • Develops and executes contracts and agreements for contractors, fiduciary, and hotel or motel vouchers.
  • Collaborates with key stakeholders and builds strong working relationships.
  • Oversee and facilitate Tenant-Based Rental Assistance (TBRA) agreements to calculate rent proportions, facilitate the rental agreement process, prepare the rental agreements, and provide client education on rights and responsibilities as tenants.
  • Oversee and facilitate the Short-term Rent, Mortgage, and Utility (STRMU) process to include a commitment to pay letters, working with case managers, landlords, utility companies, and clients throughout the process.
  • Serve as a contact between the Communicable Disease Treatment Program to Enterprise Technology Services (ETS) for CDU Treatment program systems.
  • Train new staff and program stakeholders using CDU Treatment systems.
  • Oversee the clinical quality management for the Communicable Disease Treatment Program
  • Prepares written and verbal reports on program performance.QualificationsPREFERENCES:Preference may be given to candidates with a Bachelor's degree in social work, business administration, or another social service degree.Preference may be given to candidates with 2 years of work experience in Benefits and Eligibility with acquired knowledge of a Senior Benefits Specialist.Preference may be given to candidates with 3-5 years of progressive work experience in housing benefit coordination, eligibility determination, and client benefit provision.KNOWLEDGE:
    • Statewide and local housing regulations and leasing procedures.
    • Knowledge of computer office programs and database systems.
    • Communication skills.
    • Basic math ability.
    • Ability to work with people with disabilities.
